How to submit your evidence

To download electronic template forms please click here. You may use other forms if they meet the recertification audit requirements outlined above.  

Your evidence must be no more than three years old.  Do not send your complete portfolio to the Council, only the relevant documents.  Do not send original documents, and please keep a copy of your evidence as any documents you send to the Council will be destroyed at the end of the audit process.
